### sec.146U Form of authority
An authority must be—
in writing in the form decided by the chairperson; and
signed by the person granting it.
An authority must state all of the following—
the name of the person granting the authority;
the date of the authority;
details of the assumed identity under the authority;
details of the evidence of the assumed identity that may be acquired under the authority;
the conditions, if any, to which the authority is subject;
why the authority is granted;
if the authority relates to an authorised identity officer—the name of the official;
if the authority relates to an authorised civilian—
the name of the authorised civilian; and
the name of the civilian’s supervisor under the authority; and
the period, of not more than 3 months, for which the authority remains in force.
The authority also must state the following—
whether it authorises an application to the independent member for a birth certificate approval;
each issuing agency to which a request may be made under section 146ZB .

- (a) whether it authorises an application to the independent member for a birth certificate approval;
