### sec.51ZF Requirements of an agreement
A care agreement must be in the approved form, signed by the parties.
A care agreement must state the following—
the name of the person in whose care the child is to be placed;
the period of the agreement;
where the child will be living;
arrangements for contact between the child and his or her parents;
the type of decisions relating to the child for which the parents must be consulted.

(sec.51ZF-ssec.1) A care agreement must be in the approved form, signed by the parties.
(sec.51ZF-ssec.2) A care agreement must state the following— the name of the person in whose care the child is to be placed; the period of the agreement; where the child will be living; arrangements for contact between the child and his or her parents; the type of decisions relating to the child for which the parents must be consulted.
