### sec.125 Application for, or renewal of, licence
An application for, or renewal of, a licence to provide care services must—
be made by a corporation; and
be made to the chief executive in the approved form; and
nominate an adult to be nominee for the licence; and
be accompanied by a notice that—
is signed by the nominee for the licence; and
states whether or not, for each person mentioned in section 126 (b) (i) or (ii) , the person holds a working with children authority or negative notice; and
if the person holds a working with children authority—states the expiry date for the authority; and
if the person does not hold a working with children authority—states whether or not the person has made a working with children check application and whether the application has been decided or withdrawn; and
if the application is for renewal of a licence—be made at least 30 days before the licence ends.
The application form approved by the chief executive may require the disclosure of the domestic violence history and traffic history of a person to whom section 142A (a) applies.

- (d) be accompanied by a notice that— (i) is signed by the nominee for the licence; and (ii) states whether or not, for each person mentioned in section 126 (b) (i) or (ii) , the person holds a working with children authority or negative notice; and (iii) if the person holds a working with children authority—states the expiry date for the authority; and (iv) if the person does not hold a working with children authority—states whether or not the person has made a working with children check application and whether the application has been decided or withdrawn; and
- (i) is signed by the nominee for the licence; and
- (ii) states whether or not, for each person mentioned in section 126 (b) (i) or (ii) , the person holds a working with children authority or negative notice; and
- (iii) if the person holds a working with children authority—states the expiry date for the authority; and
- (iv) if the person does not hold a working with children authority—states whether or not the person has made a working with children check application and whether the application has been decided or withdrawn; and
- (e) if the application is for renewal of a licence—be made at least 30 days before the licence ends.
- (i) is signed by the nominee for the licence; and
- (ii) states whether or not, for each person mentioned in section 126 (b) (i) or (ii) , the person holds a working with children authority or negative notice; and
- (iii) if the person holds a working with children authority—states the expiry date for the authority; and
