### sec.156 Requirements for licence application
The application must—
be in the approved form; and
state the level of licence applied for; and
be accompanied by each of the following—
the application fee prescribed under a regulation;
the licence fee for the licence prescribed under a regulation;
evidence of the applicant’s identity;
a copy of the applicant’s certificate of accreditation from an accreditation standards body for the level of licence applied for.

- (a) be in the approved form; and
