### sec.21 Applying for registration as a service provider
An application for registration as a service provider must be—
made to the regulator in the approved form; and
supported by sufficient information to enable the regulator to decide the application; and
accompanied by—
the fee prescribed under a regulation; and
if the applicant is the prescribed related entity of the relevant infrastructure owner—the owner’s written consent to the registration of the prescribed related entity.
The regulator may require either or both of the following to give additional information about the application—
the applicant;
if the applicant is the prescribed related entity of the relevant infrastructure owner—the owner.
The regulator may require the information included in the application, or the additional information required under subsection (2) , to be verified by statutory declaration.

(sec.21-ssec.1) An application for registration as a service provider must be— made to the regulator in the approved form; and supported by sufficient information to enable the regulator to decide the application; and accompanied by— the fee prescribed under a regulation; and if the applicant is the prescribed related entity of the relevant infrastructure owner—the owner’s written consent to the registration of the prescribed related entity.
(sec.21-ssec.2) The regulator may require either or both of the following to give additional information about the application— the applicant; if the applicant is the prescribed related entity of the relevant infrastructure owner—the owner.
(sec.21-ssec.3) The regulator may require the information included in the application, or the additional information required under subsection (2) , to be verified by statutory declaration.
- (a) made to the regulator in the approved form; and
- (b) supported by sufficient information to enable the regulator to decide the application; and
- (c) accompanied by— (i) the fee prescribed under a regulation; and (ii) if the applicant is the prescribed related entity of the relevant infrastructure owner—the owner’s written consent to the registration of the prescribed related entity.
- (i) the fee prescribed under a regulation; and
