### sec.365Q Application for relief under miscellaneous provisions of Commercial Arbitration Act 2013
An application for relief under the Commercial Arbitration Act 2013 , section 11 (3) or (4) , 13 (4) , 14 , 16 (9) , 17H , 17I , 17J , 19 (6) or 27 must be in the approved form.
The application must be accompanied by an affidavit stating the material facts on which the application for relief is based.

(sec.365Q-ssec.1) An application for relief under the Commercial Arbitration Act 2013 , section 11 (3) or (4) , 13 (4) , 14 , 16 (9) , 17H , 17I , 17J , 19 (6) or 27 must be in the approved form.
