### sch.1A-sec.12.2 Application for summons for appearance of person ( Corporations Act , s 1071D (4) )—form 18
An application for the issue of a summons under the Corporations Act , section 1071D (4) must be made by filing an originating application or an interlocutory application.
The application may be made ex parte.
The originating application, or interlocutory application, seeking the issue of the summons must be—
supported by an affidavit stating the facts in support of the application; and
accompanied by a draft summons.
Unless the court otherwise orders, a summons issued under this rule must be in form 18.

(sch.1A-sec.12.2-ssec.1) An application for the issue of a summons under the Corporations Act , section 1071D (4) must be made by filing an originating application or an interlocutory application.
