### sec.282 Application to chief executive for information
A person may apply in writing to the chief executive to be given health information held by a health agency for research being conducted by the person or by an entity of which the person is a member.
The application must state the following—
a description of the research that includes—
the purpose of the research; and
the methodology of the research;
the type of information required;
the reasons the information is required;
how the privacy of any individual identified will be protected;
if the information will be needed at intervals during the research, details of the intervals;
the name of the person or entity proposing to conduct the research;
the names of all persons who will be given the information for the research;
the duration of the research;
the views of a human research ethics committee about the research including contact details for the committee.

- (a) a description of the research that includes— (i) the purpose of the research; and (ii) the methodology of the research;

- (b) the type of information required;
- (c) the reasons the information is required;
- (d) how the privacy of any individual identified will be protected;
- (e) if the information will be needed at intervals during the research, details of the intervals;
- (f) the name of the person or entity proposing to conduct the research;
- (g) the names of all persons who will be given the information for the research;
- (h) the duration of the research;
- (i) the views of a human research ethics committee about the research including contact details for the committee.
