### sec.714 Approved forms
A form may be approved under this Act.
The form may be approved by—
if the form is to be used for an application to or another purpose relating to the Supreme Court, or for a costs assessment—the rules committee within the meaning of the Supreme Court of Queensland Act 1991 , section 89 ; or
if the form is to be used for an application to or another purpose relating to the committee—the chairperson of the committee; or
if the form is to be used for an application to or another purpose relating to a regulatory authority, however described—the regulatory authority; or
if the form is to be used for a purpose relating to the commissioner—the commissioner; or
if the form is to be used for an application to or another purpose relating to the board—the board.
If a provision provides that an approved form may provide for a matter, however expressed, the provision does not limit other matters that may be included in the form.
Subsection (2) is subject to a provision that expressly provides who may approve a form.

(sec.714-ssec.1) A form may be approved under this Act.
(sec.714-ssec.2) The form may be approved by— if the form is to be used for an application to or another purpose relating to the Supreme Court, or for a costs assessment—the rules committee within the meaning of the Supreme Court of Queensland Act 1991 , section 89 ; or if the form is to be used for an application to or another purpose relating to the committee—the chairperson of the committee; or if the form is to be used for an application to or another purpose relating to a regulatory authority, however described—the regulatory authority; or if the form is to be used for a purpose relating to the commissioner—the commissioner; or if the form is to be used for an application to or another purpose relating to the board—the board.
(sec.714-ssec.3) If a provision provides that an approved form may provide for a matter, however expressed, the provision does not limit other matters that may be included in the form.
(sec.714-ssec.4) Subsection (2) is subject to a provision that expressly provides who may approve a form.

- (c) if the form is to be used for an application to or another purpose relating to a regulatory authority, however described—the regulatory authority; or
- (d) if the form is to be used for a purpose relating to the commissioner—the commissioner; or
- (e) if the form is to be used for an application to or another purpose relating to the board—the board.
