### sec.94 Definitions for div 2 In this division— blameworthy act means any of the following— an intentionally unsafe act; deliberate patient abuse; conduct that constitutes a criminal offence. chain of events document ... s 94 def chain of events document om 2014 No. 65 s 22 (1) commissioning authority see section 98 . coroner see the Coroners Act 2003 , schedule 2 . health service facility means— a public sector health service facility; or a private health facility; or a prescribed health service facility. s 94 def health service facility amd 2014 No. 65 s 22 (3) prescribed health service facility means a facility— at which a health service is provided; and that is prescribed by regulation as a prescribed health service facility. s 94 def prescribed health service facility ins 2014 No. 65 s 22 (2) RCA , of a reportable event, see section 95 . RCA report see section 100 (1) .
