### sec.94 Definitions for div 2
In this division—
blameworthy act means any of the following—
an intentionally unsafe act;
deliberate patient abuse;
conduct that constitutes a criminal offence.

commissioning authority see section 98 .
coroner see the Coroners Act 2003 , schedule 2 .
health service facility means—
a public sector health service facility; or
a private health facility; or
a prescribed health service facility.

prescribed health service facility means a facility—
at which a health service is provided; and
that is prescribed by regulation as a prescribed health service facility.

RCA , of a reportable event, see section 95 .
RCA report see section 100 (1) .
RCA team means a group of persons appointed under section 98 .
relevant health service , for a reportable event, means the health service during the provision of which the reportable event happened.
reportable event —
generally—means an event prescribed under a regulation that happens while a health service is being provided; or
in relation to an RCA report—means the reportable event to which the report relates.

root cause analysis , of a reportable event, see section 95 .

- (a) an intentionally unsafe act;
- (b) deliberate patient abuse;
- (c) conduct that constitutes a criminal offence.
- (a) a public sector health service facility; or
- (b) a private health facility; or
- (c) a prescribed health service facility.
- (a) at which a health service is provided; and
- (b) that is prescribed by regulation as a prescribed health service facility.
- (a) generally—means an event prescribed under a regulation that happens while a health service is being provided; or
- (b) in relation to an RCA report—means the reportable event to which the report relates.
