### sec.124 Form of licence
A supplier’s licence must be in the approved form.
The approved form must provide for the inclusion of the following particulars—
the name and address of the holder of the licence;
the date of issue of the licence;
the expiry date of the licence;
the conditions of the licence;
other particulars prescribed under a regulation.

(sec.124-ssec.1) A supplier’s licence must be in the approved form.
