### sec.115 Application for licence
An application for a supplier’s licence may be made only by a body corporate.
An application must—
be made to the commissioner; and
be in the approved form.
An application must be accompanied by the following—
a copy of the certificate of registration or incorporation as a body corporate of the applicant;
a copy of the memorandum and articles of association, rules, constitution or other incorporating documents of the applicant, in force when making the application;
a copy of the last audited balance sheet or statement of the financial affairs of the applicant;
a disclosure affidavit;
Section 182 deals with the requirements for disclosure affidavits.

The copy of a document mentioned in subsection (3) (b) or (c) must be certified as a true copy by the secretary of the applicant or another person authorised by the applicant to make the certification.
