### sec.91 Giving information to administering authority in prescribed form
The chief executive must prepare forms (each a prescribed form ) for use in giving information to the administering authority under division 3 .
Each form mentioned in section 75 (a) or (b) must be marked with a unique identifier consisting of numbers, letters or both.
If a waste handler gives information in a form mentioned in section 75 (c) , the waste handler must ensure a unique identifier (consisting of numbers, letters or both) is stated for each load of trackable waste mentioned in the form.
The chief executive must make the forms available to the public, whether published on the department’s website or otherwise, on payment of a reasonable fee.
If a person obtains a prescribed form from the chief executive and uses it to give information to the administering authority under division 3 about the transportation of trackable waste, the number marked on the form under subsection (2) is the unique identifier for the load of waste.
