### sec.435M Form of application
An application must be made in the approved form.
The following applications for approval may be combined—
an application for approval of a biosecurity accreditation scheme;
an application by the owner of the scheme to be an approved operator of the scheme.
If any of the following persons have a conviction for a relevant accreditation offence, other than a spent conviction, the application must include details of the offence and the circumstances of its commission—
the applicant;
for an applicant that is a corporation—an executive officer of the corporation;
for an applicant that is an incorporated association—a member of the association’s management committee.
Subsection (3) does not limit the information that may be required under the approved form.

(sec.435M-ssec.1) An application must be made in the approved form.

- (a) an application for approval of a biosecurity accreditation scheme;
- (b) an application by the owner of the scheme to be an approved operator of the scheme.
