### sec.115 Obligation to notify chief executive of changed or new information relevant to eligibility or suitability
This section applies if—
the person has given information about a matter to the chief executive under this part or part 4 or 5 ; and
the person becomes aware that the information has changed or becomes aware of new information relating to the matter; and
the changed or new information is relevant to the person’s eligibility to be assessed under this part or suitability to be an adoptive parent.
The person must immediately give the chief executive a notice of the changed or new information.
Without limiting subsection (1) , the person must give notice of a change in—
the person’s personal history; or
the membership of the person’s household; or
information of which the person is aware, or that the person reasonably suspects, about the personal history of an adult member of the person’s household.
A reference in this section to a change in a person’s personal history includes, for a person with no personal history, the acquisition of a personal history.

(sec.115-ssec.2) The person must immediately give the chief executive a notice of the changed or new information.

- (a) the person has given information about a matter to the chief executive under this part or part 4 or 5 ; and
- (b) the person becomes aware that the information has changed or becomes aware of new information relating to the matter; and
- (c) the changed or new information is relevant to the person’s eligibility to be assessed under this part or suitability to be an adoptive parent.
- (a) the person’s personal history; or
- (b) the membership of the person’s household; or
- (c) information of which the person is aware, or that the person reasonably suspects, about the personal history of an adult member of the person’s household.
