and\n\n(b) excludes heritable and congenital forms of hypopituitarism.\n\n> Note 1: Pituitary hormones are growth hormone, follicle stimulating hormone (FSH), luteinising hormone, adrenocorticotropic hormone (ACTH), thyroid stimulating hormone (TSH), prolactin, oxytocin and antidiuretic hormone (ADH).\n\n> Note 2: The clinical presentation of hypopituitarism can be acute or chronic, and the order and amount of the specific hormone deficiency depends on the nature and speed of damage to the hypothalamic–pituitary region.\n\n(3) While hypopituitarism attracts ICD‑10‑AM code E23.0, in applying this Statement of Principles the meaning of hypopituitarism is that given in subsection (2).\n\n(4) For subsection (3), a reference to an ICD-10-AM code is a reference to the code assigned to a particular kind of injury or disease in The International Statistical Classification of Diseases and Related Health Problems, Tenth Revision, Australian Modification (ICD-10-AM), Tenth Edition, effective date of 1 July 2017, copyrighted by the Independent Hospital Pricing Authority, ISBN 978-1-76007-296-4.\n\nDeath from hypopituitarism\n\n(5) For the purposes of this Statement of Principles, hypopituitarism, in relation to a person, includes death from a terminal event or condition that was contributed to by the person's hypopituitarism.\n\n> Note: terminal event is defined in the Schedule 1 – Dictionary.\n\n8 Basis for determining the factors\n\nThe Repatriation Medical Authority is of the view that there is sound medical‑scientific evidence that indicates that hypopituitarism and death from hypopituitarism can be related to relevant service rendered by veterans, members of Peacekeeping Forces, or members of the Forces under the VEA, or members under the MRCA.\n\n> Note: MRCA, relevant service and VEA are defined in the Schedule 1 – Dictionary.\n\n9 Factors that must exist\n\nAt least one of the following factors must as a minimum exist before it can be said that a reasonable hypothesis has been raised connecting hypopituitarism or death from hypopituitarism with the circumstances of a person's relevant service:\n\n(1) having an autoimmune disorder involving the pituitary gland at the time of the clinical onset of hypopituitarism;\n\n(2) having a disorder from the specified list of infiltrative, inflammatory or granulomatous disorders, involving the pituitary gland or hypothalamus, at the time of the clinical onset of hypopituitarism;\n\n> Note: specified list of infiltrative, inflammatory or granulomatous disorders is defined in the Schedule 1 - Dictionary.\n\n(3) having infection with human immunodeficiency virus before the clinical onset of hypopituitarism;\n\n(4) having a viral, bacterial, fungal or protozoal infection of the pituitary gland, hypothalamus, brain or cerebral meninges, within the five years before the clinical onset of hypopituitarism;\n\n(5) having Hantavirus haemorrhagic fever with renal syndrome within the five years before the clinical onset of hypopituitarism;\n\n> Note: Hantavirus haemorrhagic fever with renal syndrome is defined in the Schedule 1 \\- Dictionary.\n\n(6) having moderate to severe traumatic brain injury within the ten years before the clinical onset of hypopituitarism;\n\n(7) having concussion within the five years before the clinical onset of hypopituitarism;\n\n(8) participating in a high impact contact activity involving blows to the head:\n\n(a) on at least 50 occasions within a continuous period of 20 years; The 2022 amendments updated specific medical factors (particularly around immune checkpoint inhibitors) to reflect evolving medical knowledge, but did not expand the scope beyond determining service connection for hypopituitarism in the veterans compensation context. The structure remains focused on the core function of establishing reasonable hypotheses for service-related causation."},"complexity_factors":["Moderate length (approximately 37 enumerated factors across onset and worsening scenarios)","Multiple defined terms in Schedule 1 Dictionary (approximately 10-12 substantive definitions)","Parallel structure requiring careful tracking — factors 1-18 for clinical onset, factors 19-36 for clinical worsening, plus factor 37 for clinical management","Cross-references to other Statements of Principles in section 11","Specific temporal requirements (5, 6, 10, 20-year windows) that must be precisely calculated","Medical terminology requiring definitions (e.g., 'lymphocytic hypophysitis', 'immune checkpoint inhibitor', 'Hantavirus haemorrhagic fever with renal syndrome')","Conditional logic in factor 8 (high impact contact activity) requiring both frequency (50 occasions) and duration (20-year continuous period) thresholds","Exclusion clause in section 7(2)(b) for heritable/congenital forms"],"plain_english_summary":"This Statement of Principles sets the medical rules for linking hypopituitarism to military service for claims under the Veterans' Entitlements Act and the Military Rehabilitation and Compensation Act. It establishes a \"reasonable hypothesis\" test: if one of 37 listed medical factors (e.g., traumatic brain injury, infection, brain surgery, or immune checkpoint inhibitor therapy) can be connected to service, the condition may be presumed service-related.\n\nIt affects current and former ADF members, veterans, peacekeepers and their families, and determines access to compensation and treatment; hereditary forms are excluded and many factors have time limits."}},"importantCases":[],"_links":{"self":"/api/acts/statement-of-principles-concerning-hypopituitarism-reasonable-hypothesis-no-11-of-2019","history":"/api/acts/statement-of-principles-concerning-hypopituitarism-reasonable-hypothesis-no-11-of-2019/history","analysis":"/api/acts/statement-of-principles-concerning-hypopituitarism-reasonable-hypothesis-no-11-of-2019/analysis","conflicts":"/api/acts/statement-of-principles-concerning-hypopituitarism-reasonable-hypothesis-no-11-of-2019/conflicts","importantCases":"/api/acts/statement-of-principles-concerning-hypopituitarism-reasonable-hypothesis-no-11-of-2019/important-cases","documents":"/api/acts/statement-of-principles-concerning-hypopituitarism-reasonable-hypothesis-no-11-of-2019/documents"}}
