This Determination may be cited as the Private Health Insurance Incentives (Form and Content of Receipt) Determination.\n\nCommencement\n\n2. This Determination is to commence on 1 January 2000.\n\nInterpretation\n\n3. In this Determination:\n\n“Act” means the Private Health Insurance Incentives Act 1998.\n\n“health fund” has the same meaning as assigned in the Private Health Insurance Incentives Act 1998.\n\nForm of Receipt\n\n4. For the purposes of section 4-6 of the Act, a receipt from a health fund must be in either written or printed form.\n\n  \n\nContent of Receipt\n\n5. For the purposes of section 4-6 of the Act, a receipt from a health fund must contain the following information:\n\n(a) Fund name;\n\n(b) Fund identification;\n\n(c) Fund membership number;\n\n(d) amount of premium paid (related to the period of cover being claimed);\n\n(e) period of cover paid (from and to dates); and\n\n(f) a sentence saying ‘The premium paid for this policy, as shown above, has not been reduced by the Federal Government’s 30% Rebate’ or the amount of rebate deducted","sortOrder":0}],"analysis":{"kimi_summary":{"_metrics":{"model":"kimi-k2.5","source":"moonshot-batch","completionTokens":2507},"content_quality":"ok","complexity_score":1,"scope_assessment":{"changed":false,"description":"This is the original 1999 Determination establishing the baseline requirements for health fund receipts.
