Investigations revealed that the fracture of the axle was due to fatigue. Two previous failures of the same axle part number occurred in 1993, which led to the issue of LBA AD D-1994-042 which mandated the replacement of part number A-511000B28B axles by improved design axle part number A-511000C28B. The actions specified by this Directive are intended to prevent main landing gear failure, which could result in loss of control of the aircraft during landing operations. |\n\n![](image.001.png) \nDavid Villiers \nDelegate of the Civil Aviation Safety Authority\n\n17 March 2009","sortOrder":0},{"sectionNumber":"AD/DO 228/16","sectionType":"part","heading":"AD/DO 228/16","content":"## AD/DO 228/16","sortOrder":1},{"sectionNumber":"Main Landing Gear Stub Axle","sectionType":"part","heading":"Main Landing Gear Stub Axle","content":"## Main Landing Gear Stub Axle","sortOrder":2},{"sectionNumber":"5/2009 DM","sectionType":"part","heading":"5/2009 DM","content":"## 5/2009 DM","sortOrder":3}],"analysis":{"kimi_summary":{"content_quality":"ok","complexity_score":2,"scope_assessment":{"changed":false,"description":"This is a standard Airworthiness Directive with a narrow, specific safety purpose: addressing a known mechanical failure mode in a particular aircraft component.
