This incident was caused by a foreign object which had penetrated inside the upper recess of the slide and made the collective lever jam. The actions specified by this AD are intended to prevent any further travel restriction caused by foreign objects, which can cause loss of collective pitch control, and subsequent loss of control of the helicopter. |\n\n![](image.001.png) \nCharles Lenarcic \nDelegate of the Civil Aviation Safety Authority\n\n21 January 2008","sortOrder":0},{"sectionNumber":"AD/AS 355/97","sectionType":"part","heading":"AD/AS 355/97","content":"## AD/AS 355/97","sortOrder":1},{"sectionNumber":"Rotor Flight Control - Collective Lever ","sectionType":"part","heading":"Rotor Flight Control - Collective Lever Recess","content":"## Rotor Flight Control - Collective Lever Recess","sortOrder":2},{"sectionNumber":"3/2008","sectionType":"part","heading":"3/2008","content":"## 3/2008","sortOrder":3}],"analysis":{"flash_summary_failed":{"failed":true,"reason":"A positive credit balance is required for all requests, including BYOK, so fallback providers remain available.
